In order to generate physical forms to take to your physician, you must begin by registering in Final Forms. The program will generate the needed forms. For more information and instructions, go to our school website and click on the"Parents/ Community" tab at the top. In the drop-down click on Final Forms to get started.

Final Forms is the online data management system that we use to manage emergency medical forms, handbooks, athletic forms, and other activities for students. Since forms have previously been filled out, for the 2020-21 school year parents must log into their Final Forms account to review the forms, verify information is current, and provide their signature at the bottom of each form. Students in grades 5-12 must also log into their student account and sign necessary forms.
For students in grades 7-12 that plan to play a fall sport, the parent and student registration needs to be completed as soon as possible in order to begin fall practices.
Tips to remember when updating your forms::
For students in grades PK-4, student emails and student signatures are NOT required.
For students in grades 5-12, the student email address must be the school email address (email@example.com). A registered parent email cannot be used as a student email address.
Grades 7-12: If you are a student athlete and your OHSAAPhysical has been completed, you still must SIGN the OHSAAPPE Medical History and the OHSAAPPE Physical (for Physicians), but you will NOT need to print them. If your physical has not been completed, you will print these forms to take with you to the physician’s office.
